LPN Care Coordinator/ Assistant Case Management at WBAMC
Care Coordination/ Case Manager Assistant duties: 1. Determine if soldier is progressing in accordance with treatment plan and notify Supervisor Case Manager and Command when solider is not progressing with treatment plan. 2. Analyze information received from the patient and healthcare team to identify barriers to progress. 3. Reinforce goals for healing and transition with the soldier, healthcare team, and Command. 4. Coordinate with members of the healthcare team and unit leadership to ensure appropriate and timely treatments are delivered in order to promote recovery. 5. Obtain updates of patient status from hospitalized sites and communicates to the supervisor or appropriate clinical service to coordinate patient care needs. 6. Assist in coordinating and obtaining services and resources needed by the soldier. 7. Identify soldiers appropriate for referral to the WTU and facilitates entry through the Triad of Leadership. 8. Track lists of appointments/ missed appointment statistics for the MMC. 9. Manages a caseload of 30-40 patients monthly
