Conduct telephone outreach to membership to gather monthly updates of members health and psychosocial status
Manage clear communication with primary care managers regarding significant changes of member health condition
Ensure that state requirement of monthly member outreach is completed for assigned members
Meet productivity metrics as assigned by upper management
Conduct 45 outreach calls and a minimum of 15 completed assessment calls per day
Properly document member contacts in the electronic medical record for primary care manager use to create care plans
Create job aid for letter management which outline letter templates
Schedule home visits for members with nurses
Respond to member questions and resolve discrepancies
Research and enter members in Uniformed Assessment System
Assist with data entering correspondence for Care Team
Training new hires on in house responsibilities and letter management
Follow established routing of member questions and/or concerns to proper departments
Involved in the call center which dealt with calls from patients, nursing staff and physicians
Received and processed referrals from doctors order in a timely manner
Assisted with scheduling and updating patient clinic appointments
Completed processing pre-registration and eligibility screenings
Verified insurance, pre-authorizations and financial counseling
Interpreted for patients, nurses and doctors
