I have completed Activity Duty obligations for the United States Navy as a Hospital Corpsmen in which I was assigned to the Emergency Department at the Robert E. Bush Naval Hospital in Twentnine Palms, CA. I have also graduated with a Bachelors of Science in Health Science and a minor in Psychology. Furthermore I have obtained a Master of Arts in Administrative Science and a Master of Science in Healthcare Administration & Leadership. I am currently working for the Department of Veterans Affairs Health Administration at the Aleda E. Lutz VAMC as a Fee Basis Program Specialists.

Review, analyze, abstract, compile, maintain, and extract information from the health record. Check for completeness and accuracy of the health record, and monitor and report for regulatory compliance. Serve as a Technical expert in health record content and documentation requirements. Responsible for ensuring that all patient care data entered Computerized Patient Record System (CPRS) is accurate, timely, and completed. Adhere to established documentation requirements outlined by The Joint Commission regulations, Veterans Health Administration (VHA) guidelines and medical-legal requirements. Review records for adherence with CMS guidelines related to student, resident supervision and attending physician presence and documentation. Review health records to ensure that all records contain sufficient information to meet medical/legal requirements. Perform quantitative and qualitative analysis on both computer and paper base records. Identifies health record deficiencies, tracks deficiencies and ensures that the appropriate individual completes their deficiencies. Assist physicians with the completion of delinquent/incomplete health records and serves as the expert resource for clinical staff regarding requirement for complete documentation. Assess paper health records and electronic Computerized Patient Record System (CPRS) for accuracy and note deficiencies. Prepare complex weekly and monthly reports noting the status of all incomplete/delinquent records and compliance reports and submits the reports to the appropriate administrative and clinical staff. Develop and compile written reports trending provider specific, service, and hospital compliance reports regarding admission subsequent and discharge notes.
