Complete admission paperwork with new residents and/or their families.
Ensure/complete a Preadmission Screening and Resident Review (PASRR), a federal requirement to help ensure that individuals are not inappropriately placed in nursing homes for long term care, for each resident in the facility.
Complete Minimum Data Set (MDS) assessments during the Assessment Reference Date (ARD) and participate in care plans for all residents in the 85-bed facility.
Assist with room changes, deciding what rooms would fit residents best, and investigations concerning possible abuse.
Attend Care Conferences for all residents in the facility to communicate how things are going with the resident and their families.
Ensure residents have any external agencies in place when discharging from the facility to ensure a smooth transition home.
