Social Worker II at Cumberland County Department of Public Health

I currently work with the Care Coordination for Children (CC4C) program for families. As a care manager, I provide support to families with children ages birth to 5 years of age. I link the parent, child and family to medical care, discuss the child's special healthcare needs, teach parents how to care for their newborn and/or children and provide information on community resources and their child's stages of growth and development.
