As a social worker with PACE: I assist in providing supportive care to participants and caregivers through care transitions by offering counseling and resources for caregiver support to decrease caregiver burden. As well as by facilitating family care plan meetings to ensure participant and family expectations are addressed. Ensuring families and participates are educated and able to access community services in order to increase participants’ overall quality of life. I assist in connecting participants and families to community experts in Elder Law and safety, guardianship, and power of attorney as well as coordinate hospital discharges to include family readiness, transportation, home health, and durable medical equipment. As the Electronic Medical Record System Administrator: I train new employees to accurately use electronic medical record system and co-develop and improve processes and protocol to improve utilization management. In addition, I complete monthly record audits to ensure interdisciplinary team assessments are completed according to CMS regulations.
