○ Effectively communicate with nurses, other pharmacists, physicians, patients, and caregivers ○ Function as drug information consultant especially dosing of medications and appropriate therapy recommendation for treatment of various disease states using evidenced-based literature ○ Accurately calculate patients’ antibiotics doses including vancomycin ○ Performed daily pain rounding, and counsel patients on their pain medications in a simplified language while also validating patients understanding of the information. ○ Respond to “Code Blue, code sepsis, and code STEMI on several occasions ○ Proficient in technician duties such as chemotherapy preparation, unit dosing, and intravenous medication preparations ○ Accurately managing and monitoring drug therapy for patients on an anticoagulation regimen and deep vein thrombosis prophylaxis based on hospital protocol. ○ Provides an effective answer to physician's questions regarding drug-disease, and drug-drug interactions using evidenced-based literature and guidelines in a timely manner. ○ Counsel patients on potential adverse drug reactions on several medications and specific key points to remember on certain medications based on their disease state and renal functions ○ Conducted a motivational interview to patients that are willing or thinking of quitting smoking prior to being discharged from the hospital.

To increase my knowledge in pharmaceutical studies by doing lots of Pharmaceutical researches so as to have more understanding is all kinds of pharmacy practices, and as well as making impact in patient's care by providing an individualized pharmaceutical care plan that will meet the patient's goal and improve their quality of life.
