Cassandra Plamondon

• Assists nursing director in the management of 36 bed med-surg tele unit • Serves as a clinical resource and provides assistance to other patient care providers. • Functions as a central resource with issues concerning clinical and support services, patient and family complaints/issues, clinical and non-clinical emergent issues, and systems breakdowns. • Facilitates staff coverage, coordinate patient care assignments, and accepts sick calls. Adjusts patient care assignment as needed. • Collaborates with admissions facilitator to manage beds and unit census. • Plans for discharges and admissions in collaboration with case management, physicians, social worker, and PT/OT. • Responsible for maintaining emergency medical equipment. • Responds to all triggers and emergency situations on unit. • Supports and mentors newly hired nurses and personnel. • Attends required leadership meetings. • Involved in quality improvement initiatives on the unit.

• Staffs floor one to two days a week to an assigned four patient-assignment on a 36 bed med-surg tele unit. • Provides direct care to patients and makes appropriate nursing judgment calls as needed • Coordinates the care of patients and delegates appropriate tasks to unlicensed assistive staff members in order to provide safe, effective, and efficient care. • Develops and adjusts 24hour care plans according to patient’s change in medical status • Identifies resources necessary to implement patient care. • Collaborates with other health care professionals to determine the health care needs of patients. • Effectively communicates the plan of care with patient, families, and the multidisciplinary team. • Maintains annual mandatory education requirements. • Participates in and encourages quality improvement activities on the unit. • Assists and advises newly hired nurses and personnel. • Adheres to unit and hospital policies and procedures while providing direct patient care.
