As a prevention services case planner in the clinical unit, responsibilities include weekly contact with families and regular collateral contact to ensure compliance with other service providers. Responsibilities also include organizing and leading case conferences and completing family assessment and service plans (FASP). The case planner assists families with establishing stable housing and food supply, accessing medical and mental health care services, and accessing social services and community resources with the ultimate goal of preventing children's placement in foster care or reunification of children already in placement.
Assist Case Planner with coordinating prevention services, allowing parents to develop safe and comfortable environments for their children. Responsibilities include supervising visits between family members, transporting clients, assisting clients with accessing medical, mental health, and social services, and completing appropriate documentation of services.
Aide other volunteers and counselors in setting up and cleaning up activities. Assist campers with completing activities adapted to their needs; all campers are children between the ages of 6 and 16 with serious medical diagnoses that would otherwise prevent them from attending summer camp.
Responsibilities include assisting medical staff by stocking supplies, walking patients, and meeting patients basic needs. Additional responsibilities include organizing paperwork, answering the phone, and other clerical tasks.
