Inpatient Care Transitions Program Manager, Complex Care & Post-Acute Strategy at Brigham and Women's Hospital

Assist the Complex Care Team in executing the strategic vision of Care Continuum Management (CCM) related to the management of high-risk patients across the continuum and the Brigham Health post-acute strategy. CCM is a multi-disciplinary pursuit whose mission is to ensure that all patients receive high quality and safe care that is seamless and coordinated across different locations and occurs in the most appropriate care setting for a given patient and/or population. High-risk Care Management involves supporting patients who require additional coordination of care due to their clinical complexity, psychosocial needs or financial vulnerability. The post-acute strategy ensures that Brigham Health patients who require any type of post-acute care are appropriately identified and can access the highest quality post-acute care in a seamless manner.

Case Manager, Network Float, Clinical Specialist at Spaulding Rehabilitation Network

Functional role as a Case Manager throughout the Spaulding Rehabilitation Network with assigments at the various sites (SRH, SHC, SRB, and SCC) for periods of various durations based on staffing needs at that location. Expert across all levels of care including IRF, LTAC, and SNF with the ability to provide support and coverage at all settings of patient care. Provides mentoring, support and training to new Case Managers hired at any site.

Assist in operationalizing regulatory and organizational changes to workflow and processes while advising the Case Management Senior Directors at each facility around standardization of case management process across the network. Support the assessment of clinical skills and competence of new staff to provide input as to the department managers for the purposes of annual performance evaluations.

Serves as a member of the SRN CM/SW Education Committee to support educational activities and professional development activities for the network.
