Clinical liaison at Transitions LifeCare (founded as Hospice of Wake County in 1979)
Act as hospice case manager for hospice patients in the hospital; coordinate Hospice plan of care with hospital staff and Hospice IDG members. Provide education and support to patients, families, facility staff, and referral sources re Hospice services and end of life care. I partner with Provider Relations to implement results oriented, innovative education, customer service, account management, and marketing strategies within the hospital(s) assigned to promote timely, appropriate patient referrals and excellence in end of life care.
