Adult/Gerontological nurse practitioner who works with an interprofessional team to provide medical and nursing care for the aging in the community. Program Development My role is diverse and I have a strong interest in program development. My role has allowed me to start a transitional care home based program which functions to provide care for the high risk patient populations in Forsyth county and surrounding communities and I am also directer over the house call program at Wake Forest Baptist Health. Patient Advocate In addition to program development, I enjoy providing holistic medical and nursing care to geriatric patients in the community both in a clinic and home setting. My role allows me to connect patients to community agencies as appropriate and also provide social and functional services for the aging in the community. Taking care of patients in their homes allows me to fully understand the complex relationship between their everyday lives and their health. Advocate in the Workplace There are many exciting changes in my role, which has allowed me to be active on a hospital wide committee that provides counsel to Executive Leadership for workplace improvement for all employees, while also functioning as a NP and PA advocate on counsels in the workplace. Volunteer Clinic Director In my spare time, I founded a free health care clinic (Grace Clinic at New Light Missionary Baptist Church) and serve as a volunteer clinical director on a monthly basis.

Work with a team of Geriatricians and nurses to provide geriatric focused care in the community, nursing home, and clinic setting. Serve a primary care provider for the geriatric population. Perform Geriatric consults for community primary care providers who need to patients to be seen for geriatric specific care. Consult in leadership role of Geriatric clinic at Wake Forest Baptist Hospital and participate in developing person centered models of care for clinic and house call patients. Study clinician for dementia studies, including PACE, SNIFF, A4, and ADPPR. Active in educating and precepting medical students, nurse practitioner students, and interns. Member on the Fellowship Advisory Board, whose function is to recruit Geriatric fellows, as well as evaluate fellows on skills. I am also an active member of the Advanced Provider Leadership team whose function is to advocate for APPs in our institution. Also work with a geriatrician to run a skilled nursing unit at a local nursing home with a focus on holistic transitional care, as well as a goal of patient readmission reduction.
